Position Title: Community Support Worker

Department: Behavioral Health Services - Mobile Crisis Response (MCR)

Reports to: Mobile Crisis Response Team Lead

Position Objective and Summary:

UCAN is seeking a compassionate and dedicated Community Support Worker to join our Mobile Crisis Response (MCR) team. The Community Support Worker will provide essential support services to individuals experiencing mental health crises, ensuring they receive the necessary resources and assistance to stabilize and improve their situations.

Essential Functions and Responsibilities:

Direct Support
  • Provide immediate support and assistance to individuals in crisis.
  • Conduct home visits and community outreach to engage with clients and assess their needs.
  • Assist clients in accessing services such as housing, healthcare, and social services.
Crisis Intervention
  • Work closely with the MCR team to respond to crisis situations promptly and effectively.
  • Support the team in de-escalating crises and stabilizing individuals.
  • Help develop and implement crisis intervention plans tailored to individual needs.
Resource Coordination
  • Connect clients with appropriate community resources and services.
  • Advocate for clients to ensure they receive necessary services and support.
  • Follow up with clients to ensure continuity of care and stability.
Documentation and Reporting
  • Maintain accurate and timely documentation of all client interactions and support activities.
  • Assist in data collection and analysis to monitor program effectiveness and identify areas for improvement.
  • Prepare and submit reports to the MCR Team Lead as required.
Client Empowerment
  • Encourage and empower clients to take an active role in their recovery and stabilization.
  • Provide education and information to clients and their families about available resources and support.
  • Support clients in developing coping strategies and life skills.
Education/Job Experience/Certification:
  • Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field preferred.
  • Minimum of 2 years of experience in a community support or similar role, preferably in a mental health or crisis intervention setting.
  • Experience working with diverse populations and individuals experiencing mental health crises.
Special Knowledge and Qualifications:
  • Strong understanding of community resources and social services.
  • Excellent communication and interpersonal skills.
  • Ability to handle high-stress situations with professionalism and composure.
  • Strong organizational and multitasking abilities.
  • Proficiency in data management and report writing.

Working Environment: Must be available to work flexible hours, including evenings and weekends, as needed. Ability to travel within the service area for home visits and community outreach. Work may involve exposure to high-risk situations and emotionally charged environments.
