Community Support Worker

Prestige Healthcare Resources, Inc. is a health care company. Prestige currently provides services to over 1,500 patients in two different states and plans to scale to provide care to over 10,000 patients in five different states over the next 3-5 years. One of PHRI's major focus areas advocates and implements an "integrated care" model approach to underserved populations undergoing behavioral health related issues. The Community Support Worker is responsible for supporting consumers in attaining meaningful lives through the growth of skills necessary to live independently, complete education/training, acquire employment and have enjoyable social lives. CSWs are responsible for facilitating client's transition to independence; endorsement and application of community services. Additionally, CSWs coordinate care for client's health, housing, and vocational needs.

• Performs medically necessary community support and skill building activities identified in the client treatment/care plan
• Identify needed resources and assist consumer to obtain resources
• Provide individual interventions for the development of interpersonal and community coping skills, including adapting to home, school and work environments
• Assist and support clients in crisis situations
• Outreach clients to enhance engagement in treatment and meet "client where they are at"
• Provide services in natural settings to include; home, school, court, other social service agencies.
• Provide referrals to outside agencies
• Educate, support and consult to client's families and their support system, which is directed exclusively to the well-being and benefit of the client
• Assist the client in symptom self-monitoring and self-management for the identification and minimization of the negative effects of psychiatric symptoms which interfere with the client's daily living, financial management, personal development, or school or work performance
• Notify clinical manager when client is non-compliant with service goals

• High School Diploma or GED
• Proven experience in providing support services
• Excellent communication and interpersonal skills
• Commitment to consumer-centered care
• Valid Driver's license and access to an automobile
